Thank you for your interest in our child development centers in Leominster and Fitchburg. We hope that you will come and visit to see what our programs can offer your family on a first hand basis.
Should you be interested in enrolling your child in our program, we would like to give you an early start on the process. Below you will find a list of things that will need to be done or acquired before we can proceed with enrollment.
Call the center of your choice, or fill out an online "Request for Service" form (you can download and fax or email the RFS form located on the "Admissions Form" page). All forms that you need to complete can be found on our Web Site in the "Admissions Forms" page.
- A Medical History Record including last physical and immunizations and any pertinent medical information on your child(ren) (i.e. physical, IHCP, allergies, immunization records, IEP). Physical Exam must be within the past year period (from the date of enrollment).
- Meeting with Director/Tour school and related classroom.
- Fill out "Enrollment Book" for each child.
- Complete "Enrollment Agreement" and "Fee Agreement" with the Center Director.
- Provide a copy of a Child Care Voucher, MWCC subsidy; if applicable.
